As a courtesy to our patients, F. Lee Angus Jr. DDS will complete and file insurance forms relative to dental services. When insurance requires a predetermination of benefits before work is started, F. Lee Angus Jr. DDS will submit to the insurance company a treatment plan with charges so that you will be completely aware of your estimated coverage.
Please note, a current insurance card for each family member is needed for each visit.
